Care Coordination helps South Dakotans develop a plan for recovery from addiction. The free and confidential service is available through the Resource Hotline at 1-800-920-4343. Use this informational brochure to connect people and families in need with a Care Coordinator. This brochure details the type of support Care Coordinators are able to give and stresses how it is a free and confidential service available to anyone struggling with addiction or in need of support/guidance as a loved one of someone with addiction.
