Talent/Model Release Form
Volunteer (unpaid)

In consideration of the inestimable value of the social service programs and work that Volunteers of America providers to those who are in need in America, and in consideration of Volunteers of America allowing me to participate as a subject/talent/model (modeling) in a Volunteers of America photo shoot, I herein acknowledge that I do, freely and willingly, volunteer my modeling service for the purpose of being recorded, filmed or photographed and I waive any rights to compensation or ownership or personal use thereof. Furthermore, I understand fully that the recording, filming, photographing, for which I willingly pose will be used by Volunteers of America, for the purpose of advertising, marketing, fund development, or general promoting of the social service programs, activities, and work of Volunteers of America, its national office and affiliates.

I do hereby grant to Volunteers of America for whom I am voluntarily modeling, the irrevocable and perpetual right and permission, in respect to the recording/film/photography, that were taken of me, or in which I may be included with others, to copyright the same in its name; to use, publish, transmit, and distribute the recording/film/photography of me, individually, or in conjunction with other recording/film/photography, in whole, or in part, or reproductions thereof in color, or otherwise, and in conjunction with any of the organizations printed matter, at its offices or elsewhere, in any media now or hereafter known, including but not limited to video, film, the internet, print, radio, still photography, for the purpose of art, advertising, illustration, and trade or for any other educational, charitable, marketing, or promotional purpose, as required by Volunteers of America. I also grant Volunteers of America permission to use my name in connection with the recording/video/ photograph/film
I hereby waive any right that I may have to inspection or approve the finished product, or products, and the copy or other matter that may be used in connection therewith or the use to which it may be applied.

I hereby release and discharge Volunteers of America, its affiliates and their officers, directors and employees, the contractor/photographer, and all persons acting under Volunteers of America’s permission or authority, from any liability by virtue of any blurring, alternation, distortion or optical illusion, whether intentional or otherwise, that may occur or be produced in the taking of said recording/film/photograph or in any subsequent processing thereof, as well as any publication thereof and from any and all claims and demands arising out of, or in connection with the use of the recording/film/photograph when used for the purpose as described above, including without any limitation and all claims for the defamation, invasion of privacy, or violation of any right of publicity.

This authorization and release shall also endure to the benefit of the licensees and assigns of Volunteers of America for whom the recording/film/photography were taken.

I understand that Volunteers of America is taking these recordings/films/photographs in express reliance on this Release, and I acknowledge that Volunteers of America shall own all rights, title, and interest in and to the recordings/films/photographs taken by the recording/filming/photographing party.

I do hereby warrant that I am of full age and have the right to contract in my own name or have approval of my parent or legal guardian (as noted below).

I have read the above authorization, release, and agreement, prior to its execution, and I am fully familiar with the contents thereof.

This release shall be binding upon me and my heirs, legal representatives, and assigns.
